Welcome to Vitality Health + Wellness. By scheduling a service, purchasing a package, or engaging with our clinic in any way, you agree to the following terms and conditions. Please review them carefully.

1. Payment Terms

All services at Vitality Health + Wellness are self-pay and non-refundable.

  • Payment is due in full at the time of booking or service.

  • We accept major credit/debit cards and cash.

  • Pricing is subject to change at any time without prior notice.

2. Refunds + Cancellations

Vitality Health + Wellness does not offer refunds for services rendered, consultation fees, lab orders, injections, or custom programs.

Cancellation Policy:
Please provide 24 hours’ notice if you need to cancel or reschedule. No-shows or late cancellations may be subject to a fee or forfeiture of any prepaid services.

3. Medical Disclaimer

All services provided are intended for wellness, aesthetics, and health optimization. They do not replace your relationship with a primary care physician.

You understand that:

  • Our recommendations are based on clinical training, labs, and your health history.

  • Results may vary and are not guaranteed.

  • You are responsible for disclosing full and accurate medical history prior to treatment.

4. Service Eligibility

You must be at least 18 years old to receive services unless accompanied by a parent/legal guardian and cleared for age-appropriate care.

We reserve the right to deny services if:

  • You are not a candidate for a treatment

  • You withhold important health information

  • Your goals fall outside of our clinic’s scope of practice

5. Privacy Policy

All personal health information is protected under HIPAA and securely stored in our electronic records system. We do not sell or share your information with third parties.

6. Intellectual Property

All educational content, wellness guides, protocols, and downloadable materials provided by Vitality Health + Wellness are the intellectual property of the clinic. Reproduction or distribution without permission is prohibited.

7. Modifications

We reserve the right to update or modify these Terms at any time. Continued use of our services constitutes your agreement to any changes.
